pillsYou’ve heard of the placebo effect, right? Well, apparently it’s becoming stronger, much to the chagrin of the pharmaceuticals industry who are finding that less drugs are passing clinical trials as a result:

MK-869 wasn’t the only highly anticipated medical breakthrough to be undone in recent years by the placebo effect. From 2001 to 2006, the percentage of new products cut from development after Phase II clinical trials, when drugs are first tested against placebo, rose by 20 percent. The failure rate in more extensive Phase III trials increased by 11 percent, mainly due to surprisingly poor showings against placebo. Despite historic levels of industry investment in R&D, the US Food and Drug Administration approved only 19 first-of-their-kind remedies in 2007—the fewest since 1983—and just 24 in 2008. Half of all drugs that fail in late-stage trials drop out of the pipeline due to their inability to beat sugar pills.

The upshot is fewer new medicines available to ailing patients and more financial woes for the beleaguered pharmaceutical industry. Last November, a new type of gene therapy for Parkinson’s disease, championed by the Michael J. Fox Foundation, was abruptly withdrawn from Phase II trials after unexpectedly tanking against placebo. A stem-cell startup called Osiris Therapeutics got a drubbing on Wall Street in March, when it suspended trials of its pill for Crohn’s disease, an intestinal ailment, citing an “unusually high” response to placebo. Two days later, Eli Lilly broke off testing of a much-touted new drug for schizophrenia when volunteers showed double the expected level of placebo response.

It’s not only trials of new drugs that are crossing the futility boundary. Some products that have been on the market for decades, like Prozac, are faltering in more recent follow-up tests. In many cases, these are the compounds that, in the late ’90s, made Big Pharma more profitable than Big Oil. But if these same drugs were vetted now, the FDA might not approve some of them. Two comprehensive analyses of antidepressant trials have uncovered a dramatic increase in placebo response since the 1980s. One estimated that the so-called effect size (a measure of statistical significance) in placebo groups had nearly doubled over that time.

You’ll not find me shedding many tears for Big Pharma, frankly; as pointed out in the rest of the article, their profit margins of the last few decades has been based on turning psychiatric treatments into mental cosmetics, and the increase in the placebo response may well be tied to their incredibly effective marketing procedures – by associating drug consumption with positive images of well-being, they’ve stimulated our response to all pills, regardless of their chemical make-up. Research is increasingly suggesting that one of the biggest prerequisites for getting better is simply a reasonable expectation that one actually will. [via BoingBoing]

I’ve got something of a personal stake in this, having seen far too many friends diagnosed with one form or another of psychiatric condition and promptly put on a regime of drugs that have shattered their personalities – and lives – permanently. I’m not denying the existence of mental illness, but I’m quite convinced that overdiagnosis is rife, with many drug-treatable “syndromes” being little more than completely natural phases of mental development or reasonable responses to environmental conditions which would respond far better to counselling and emotional support. Hence I was intrigued to read an article by medical anthropologist Eugenia Tsao [via @somatosphere], who recommends that members of her profession and others should start pushing back against the relentless attempts to pathologize (and hence monetize) our reactions to our social environment:

What is revealed about a society, in which drugs are touted with increasing regularity as a treatment of choice for entirely natural responses to conditions of unnatural stress? How have we been persuaded to equate such things as recalcitrant despair (“Dysthymic Disorder,” DSM-IV-TR 300.4), adolescent rebellion (“Oppositional Defiant Disorder,” DSM-IV-TR 313.81) and social apathy (“Schizoid Personality Disorder,” DSM-IV-TR 301.20) with aberrant brain chemistry and innate genetic susceptibilities rather than with the societal circumstances in which they arise? What does it mean when increasing numbers of people feel as though they have no choice but to self-medicate with dubious chemical substances in order to stay in school, stay motivated, stay employed, and stay financially solvent?

UFO sightings coincide with UFO movies

flying saucersVia Futurismic‘s long-term good buddy Mac Tonnies come the results of an analysis of the UK MoD’s “x-files” documents, recently released to public scrutiny; apparently UFO sightings were more common around the times at which popular films or television shows featuring alien races or spacecraft were screened. The obvious conclusion here is “well, skiffy movies cause alien sightings; case closed”. But as Mac points out, that’s not logically sound:

There’s doubtlessly a correlation between science fiction and UFO reports. But while pop culture’s influence on potential UFO observers is a fascinating subject with important sociological ramifications, to flaunt Clarke’s findings as a refutation of the phenomenon in general is to willfully ignore the evidence in its entirety.

UFO researchers aren’t interested in “noise” cases — the inevitable false alarms that plague efforts to study the phenomenon (whatever its origin). Indeed, scientists who have addressed the UFO problem have always been painfully aware of the disproportionately high volume of false returns.

Now, you may more skeptical than Mac regarding the causes of UFO sightings, but his point still stands – divorce the logic from the specific subject matter, and the same applies to any sort of genuine scientific enquiry. I’m pretty sure this is what they call confirmation bias at work, and it makes me wonder how often it affects us…

… although it obviously happens often enough for it to be politically useful. 😉

Psychohistory in the real world

crowdResearchers at Indiana University believe that it may be possible to create a real-life version of Isaac Asimov’s concept of psychohistory:

Much as meteorologists predict the path and intensity of hurricanes, Indiana University’s Alessandro Vespignani believes we will one day predict with unprecedented foresight, specificity and scale such things as the economic and social effects of billions of new Internet users in China and India, or the exact location and number of airline flights to cancel around the world in order to halt the spread of a pandemic.

Psychohistory as described by Isaac Asimov holds that “while one cannot foresee the actions of a particular individual, the laws of statistics as applied to large groups of people could predict the general flow of future events.”

This certainly seems similar to the ideas of reality mining discussed here:

Vespignani writes that advances in complex networks theory and modeling, along with access to new data, will enable humans to achieve true predictive power in areas never before imagined. This capability will be realized as the one wild card in the mix — the social behavior of large aggregates of humans — becomes more definable through progress in data gathering, new informatics tools and increases in computational power.

It is an exciting direction, and offers the possibility of a black-swan style technological breakthrough. With improved data, through things like spimes and ubiquitous computing, combined with improved data processing techniques and communications there exists the possibility for a new and powerful way of studying, monitoring, and even controlling social and technological developments with precision.

Zoning out: the brain’s science fictional mode

pensive statueHere’s some good news for those of you who, like myself, are prone to losing mental focus; apparently “zoning out” is an important mental state, as well as being intrinsically linked to the way we think about the future.

The regions of the brain that become active during mind wandering belong to two important networks. One is known as the executive control system. Located mainly in the front of the brain, these regions exert a top-down influence on our conscious and unconscious thought, directing the brain’s activity toward important goals. The other regions belong to another network called the default network. In 2001 a group led by neuroscientist Marcus Raichle at Washington University discovered that this network was more active when people were simply sitting idly in a brain scanner than when they were asked to perform a particular task. The default network also becomes active during certain kinds of self-referential thinking, such as reflecting on personal experiences or picturing yourself in the future.

The fact that both of these important brain networks become active together suggests that mind wandering is not useless mental static. Instead, Schooler proposes, mind wandering allows us to work through some important thinking. Our brains process information to reach goals, but some of those goals are immediate while others are distant. Somehow we have evolved a way to switch between handling the here and now and contemplating long-term objectives. It may be no coincidence that most of the thoughts that people have during mind wandering have to do with the future.

Lunchtime doubly so

hourglassA powerfully engaging essay on the nature of mind and the perception of time over on Edge by David M. Eagleman:

Try this exercise: Put this book down [or just stop reading the screen] and go look in a mirror. Now move your eyes back and forth, so that you’re looking at your left eye, then at your right eye, then at your left eye again. When your eyes shift from one position to the other, they take time to move and land on the other location. But here’s the kicker: you never see your eyes move. What is happening to the time gaps during which your eyes are moving? Why do you feel as though there is no break in time while you’re changing your eye position? (Remember that it’s easy to detect someone else’s eyes moving, so the answer cannot be that eye movements are too fast to see.)

Not only does our perception of time vary under different conditions, different sensory inputs do not slow down to the same subjective time:

Duration distortions are not the same as a unified time slowing down, as it does in movies. Like vision, time perception is underpinned by a collaboration of separate neural mechanisms that usually work in concert but can be teased apart under the right circumstances.

This is a fascinating and SF-mineworthy area of research.
